Scope of Work:
Enter discharge record data from client-provided medical records into a spreadsheet.
Ensure all entries are complete, accurate, and consistently formatted.
Track patient information, admission/discharge dates, diagnosis, procedures, attending physician, length of stay, and follow-up details.
Flag any missing or unclear information for review.
Maintain strict confidentiality of all patient health information.

Follow-up appointment details
Delivery:
Submit a final spreadsheet that is clean, organized, and ready for review.
