Client Instruction: Medication and Prescription Records Data Entry

Overview of the Work:
You will be responsible for accurately entering patients’ medication and prescription information. This includes logging prescribed drugs, dosages, frequency, and administration instructions. Precise documentation ensures proper patient care and helps avoid medication errors. Confidentiality and attention to detail are essential.

Scope of Work:

  1. Enter all medication and prescription details into the provided system or spreadsheet.

  2. Ensure accuracy, consistency, and completeness of the data.

  3. Conduct quality checks to identify and correct any errors.

  4. Deliverables include:

    • Final Spreadsheet: Complete dataset of patient medications and prescriptions.

    • Word Report: Summary report highlighting work completed, quality checks performed, and any discrepancies.

Data Fields to be Entered:

  1. Patient Full Name

  2. Date of Birth

  3. Medication Name

  4. Dosage

  5. Frequency/Administration Schedule

  6. Prescribing Physician

  7. Start and End Dates of Medication

Client Material Source:
All source materials, including patient records and prescription forms, will be shared upon hire.

Deadline:
The completed work, including both the spreadsheet and Word report, must be submitted by 20th December.

Additional Notes:

  • Ensure confidentiality and adherence to medical data privacy standards.

  • Communicate any data inconsistencies or unclear information promptly with the client.
