You will be responsible for entering and managing patients’ medical histories. This includes past illnesses, surgeries, allergies, and family medical history. Accurate documentation ensures healthcare providers have the necessary information to make informed treatment decisions. Attention to detail and confidentiality are essential.
Scope of Work:
Enter patient medical history details into the provided system or spreadsheet.
Verify consistency and completeness of the data.
Perform quality checks to identify and correct any errors or missing information.
Deliverables include:
Final Spreadsheet: Complete dataset of patient medical histories.
Word Report: Summary report detailing the work completed, quality checks performed, and any issues encountered.

Client Material Source:
All source materials, including patient records and forms, will be shared upon hire.
Deadline:
The completed work, including both the spreadsheet and Word report, must be submitted by 20th December.
