Project Description

Client Instruction: Electronic Health Records (EHR) Data Entry

Overview of the Work:
You will be responsible for accurately entering patient information into an Electronic Health Records (EHR) system. The goal is to ensure all patient records are complete, organized, and accessible for healthcare providers. This work requires attention to detail, confidentiality, and adherence to the client’s data standards.

Scope of Work:

  1. Enter patient information into the EHR system using the source materials provided.

  2. Ensure accuracy and consistency of data across all records.

  3. Perform basic quality checks to identify and correct errors or missing information.

  4. Deliverables include:

    • Final Spreadsheet: A complete dataset containing all patient information in the client’s required format.

    • Word Report: A summary report highlighting the work completed, data quality checks performed, and any issues encountered.

Data Fields to be Entered:
The following 7 key fields must be captured for each patient:

  1. Full Name

  2. Date of Birth

  3. Gender

  4. Contact Information (phone/email)

  5. Medical History (past illnesses, surgeries, allergies)

  6. Current Medications (name, dosage, frequency)

  7. Physician Notes / Observations

Client Material Source:
All source materials, including patient records, forms, and guidelines, will be shared upon hire.

Deadline:
The completed work, including both the spreadsheet and the Word report, must be submitted by 20th December.

Additional Notes:

  • Ensure compliance with data privacy and confidentiality standards.

  • Maintain clear communication with the client regarding any ambiguities or issues during the work process.
